It's the Stors' fault as far as the shift from Duchies goes. We were all Dukedoms and Duchies, then Harald said he was king. The rest of the states, not ones to be shown up by some half-nude northerners, Declared their own territories to be kingdoms a bit later. That's how I remember it at least.

Elwynn's leader became a prince but that was unique for a while. Then Lichbrook got upgraded to a Kingdom. Then Kildare declared itself the Eastern Empire. Then Harald declared himself King. Then Goldshire acquired royal status.
Duchies are easier. Except Elwynn but that's because they're intentionally going for something different than endless feudalism

Historically, the Duke of Yardistan was also the Anarch, but this was never a de jure sort of arrangement, and the titles did not percolate up to the Landsraad. I always liked the idea that the two positions could be occupied by different people. Wasted a great opportunity for infighting.
